DIFFUSE ALVEOLAR HEMORRHAGE FOLLOWING SUGAMMADEX AND REMIFENTANIL ADMINISTRATION: A CASE REPORT.

Abstract

Background: Diffuse alveolar hemorrhage (DAH) is an uncommon but severe complication associated with various medical interventions. We present a case report of a 21-year-old male patient who developed pink frothy secretions following remifentanil administration by bolus injection along with sugammadex during emergence from general anesthesia. Case Presentation: A 21-year-old male patient with no significant medical history underwent [type of surgery]. Anesthesia was induced using [anesthetic agents], and maintenance was achieved with remifentanil and sugammadex. FESS was done. Reversal of neuromuscular blockade was accomplished with sugammadex. Upon extubation, the patient experienced sudden pink frothy secretions, respiratory distress, and decreased oxygen saturation. Management and Outcome: The patient was promptly intubated and received [specific interventions]. Diagnostic imaging, including chest x-ray and computed tomography, revealed evidence of DAH. FESS was done. Conservative care was initiated, consisting of supplemental oxygen at 4L via nasal prongs, administration of 20 mg of Lasix, and 2500 mg of tranexamic acid. Despite these measures, the patient's condition improved only partially. Discussion: This case underscores the potential association between the use of sugammadex and remifentanil and the occurrence of DAH. The exact mechanisms linking these agents to DAH warrant further investigation. Vigilance is essential, especially when dealing with patients at risk for complications. Early recognition and proper management are crucial for optimal patient outcomes. Conclusion: We report an unusual case of DAH following remifentanil administration by bolus injection and sugammadex use in a 21-year-old male patient. This case highlights the need for heightened awareness of DAH as a potential complication and emphasizes the importance of thorough monitoring and prompt intervention to address this serious condition.
