Assessment of comparison of MEWS, REMS and RAPS for Predicting Outcomes of non-Surgical Patients in Emergency Room

Abstract

Background: To compare MEWS, REMS and RAPS for predicting outcomes of nonsurgical patients in emergency room. Material and Methods: One hundred eight patients admitted in emergency department of both genders were selected. The calculation of scoring such as MEWS, REMS, and RAPS was done. Results: Out of 108 patients, males were 58 and females were 50. Diagnosis was ARDS in 12, acute gastro enteritis in 18, viral fever in 28, pneumonia in 20, URTI in 20 and others in 15 cases. Clinical characteristics showed diabetes mellitus in 26, hypertension in 30, ischemic heart disease in 14, chronic kidney disease in 16, COPD in 7 and other comorbidities in 5 cases. Event in emergency department was discharge in 13, ward admission in 30 and ICU admission in 65. Outcome was discharge in 76 and death in 32 cases. The difference was significant (P< 0.05). MEWS score >5 was seen in 20 discharged and 22 died patients and <5 seen in 556 discharged and 14 died patients. REMS score >13 seen in 10 and 8, intermediate in 16 and 14 and low in 50 and 10 discharged and died patients. The mean RAPS score was 2.1 and 4.0 in discharged and died patients. Area under the curve for MEWS, REMS score and RAPS was 0.74, 0.72 and 0.70 respectively. The difference was significant (P< 0.05). Conclusion: All scores such as MEWS, REMS, RAPS found to be equal in predicting inhospital mortality among non- surgical patients presenting to the Emergency department
