A RETROSPECTIVE STUDY OF RISK FACTORS RELATED TO ANAEMIA IN CHRONIC OSTEOMYELITIC PATIENTS ON PROLONGED LINEZOLID THERAPY

Abstract

A long-lasting infection of the bone and bone marrow brought on by bacteria, mycobacteria, or fungus is known as chronic osteomyelitis. Therefore, from a therapeutic perspective, chronic osteomyelitis presents a challenging medical situation. Beta lactam antibiotics such as penicillins and cephalosporins were frequently used to treat osteomyelitis because of their effectiveness and relative safety. The lincosamides drug like clindamycin, which is more effective, which great oral bioavailability and bone penetration. A new group of antibiotics called oxazolidinones includes linezolid is helpful for the treatment of multi drug resistant (MDR) gram positive bacteria from chronic osteomyelitis. Many of the patients with complex illnesses and the majority of patients who encountered anaemia were taking long time medications known to suppress bone marrow causing aplastic anaemia. Methods: The study was conducted at Dr. B. S. Kushwah Institute of Medical sciences, Kanpur, U.P. the data were also collected from Rama Medical College, Hospital & Research Centre from January 2022 to December 2022. The study design is a case control type of analytical study to check the toxicity of linezolid against erythrocytes. Result: Out of 180 cases with chronic osteomyelitis, 158 (87.8%) males and 42 (12.2%) females. The mean age of the participants was 53.6 year with ± 4.2 SD, received long term linezolid. Among all, 98 (54.4%) individuals had infections from orthopaedic devices. Anemia was recorded in 30 cases, which was substantially correlated with the early termination of linezolid medication. Anaemia was observed in 76 (42.2%) cases and 104 (57.8%) cases were without anaemia respectively. Factors include age, alcohol consumption, diabetes mellitus, and low pre treatment haemoglobin induced anaemia during treatment with linezolid. Conclusion : Treatment with linezolid is a significant advance in the management of chronic osteomelitis cases having MDR infection like MRSA and VRE. In this study old age patients with co morbidity were more prone to linezolid toxicity.
