A CLINICAL EVALUATION AND CORRELATION OF BITE FORCE, PERIODONTAL STATUS AND ASYMPTOMATIC TEMPOROMANDIBULAR JOINT CHANGES USING MAGNETIC RESONANCE IMAGING IN STRESSED INDIVIDUALS - A CROSS SECTIONAL STUDY

BACKGROUND: It is much evident that stress and bite force show either direct or indirect effects on periodontium whichin turn can cause changes in the temporomandibular joint tissues. Individual comparisons were executed but none of the studies were performed to find the correlation between all these parameters especially in theTMJ of asymptomatic stressed individuals using Magnetic Resonance Imaging (MRI).
OBJECTIVES: The present research aimed to evaluate and correlate the Stress, Bite Force, Clinical Periodontal status and TMJ Radiographic changes using the gold standard technique i.e. Magnetic Resonance Imaging (MRI) in order to perceive both soft and hard tissue changes of TMJ in the Asymptomatic stressed individuals.
MATERIAL AND METHODS: To categorize the stressed systemically healthy individuals from non-stressed individuals by using ISMAquestionnaire,and assess the periodontal status by using CPITN Probe, followed by bite force assessment using the Bite Force Sensor device and finally evaluate the temporomandibular joint soft and hard tissue changes by using MRI.
RESULTS: A significant correlation was seen between Bite Force and Stress condition; Gender and stress condition. Among right and left side first molar regions, males have shown a greater mean bite force on left sidethan females. No significant correlation was seen between bite force and periodontal clinical parameters (CAL, PPD& GI). However ,comparing mean bite fore on right and left sides with MRI changes of TMJ in all the views, even though no significant changes were reported, correlation were found to be negative.
CONCLUSION: Based on the preliminary results observed by clinical (stress, bite force, periodontal parameters) and MRI examinations (soft tissue changes), no direct relationship could be determined between stress and soft tissue changes of TMJ, but due to presence of mild osteoarthritic changes in the condyle in few asymptomatic subjects, these patients can be more prone to osteoarthritis condition in near future

A hospital based observational assessment of the distribution of different temporomandibular joint disorders in complete dentures patients

Aim: To determine the prevalence of temporomandibular disorders (TMD) in patients wearing complete dentures (CD).
Material and Methods: This study was carried at the department of dentistry for a period of 1 year. 100 completely edentulous patients in the age group of 22-74 years who did not complain of any TMD and were denture bearers with a varied denture wearing span, were examined for TMD signs and symptoms included in the study.
Results: Out of a total of 100 subject sex amines, the majority of the patients is 28% fell in the age group of 50-60 years, followed by 40-50 years 22%. There was no statistically significant association between different genders and age (P [χ2=1.722, P (χ2>1.722) = 0.9885. In the distribution of patients with or without TMD, 61.67% of males out of the total male subjects and 65% of females out of total female subjects showed the presence of signs. The difference was found to be statistically insignificant (P [χ2=0.347, P (χ2>0.365) = 0.5476). 42% of the subjects had 1-3 years of edentulous span followed by32%withmorethan 5 years of edentulous span and 26% of the subjects showed 3-5 years of edentulous span. 34% of the subjects showed limited mouth opening assign of TMD followed by jaw deviation which is shown by 27 (27%) of the subjects.
Conclusion: With an increase in the number of patients wearing CD, it becomes mandatory for dentists to be aware of the consequences of denture use over the anatomy and physiology of TMJ to provide quality prosthodontics treatment. The most common finding was limited mouth opening while crepitus and clicking were the least encountered

AWARENESS AMONG DENTAL STUDENTS ABOUT THE RELATIONSHIP BETWEEN MALOCCLUSION AND TMJ PROBLEMS

The aim of this study is to determine the understanding regarding the relationship between malocclusion and TMJ problems among dental students. The purpose of this study is to create awareness and determine the understanding of the relationship between malocclusion and TMJ problems among dental students. This study was conducted based on a questionnaire which consists of 10 questions through a web-linked application called Survey Monkey. A convenient sample size of 100 consecutive dental students who are currently practicing in Chennai participated in the study. As an overall result, most of the participants are aware of the relationship between malocclusion and TMJ problems. As a conclusion, awareness on the relationship between malocclusion and TMJ problems among dental students in Chennai is adequate but certain knowledge has to be brushed up among them for a higher level. Furthermore, they need to be trained on these grounds to help them treat their patients with more consent and awareness.

According to some literature data, special attention in the diagnosis of dysfunction
of the temporomandibular joint is given to a detailed analysis of complaints, the collection of
anamnesis and data of the main research methods characterizing the dental status of profile
patients in the form of structural algorithms using databases focused on this pathology [10,
12, 16, 25, 35, 37, 43], as well as generalized clinical methods for the diagnosis of functional
disorders of the dentoalveolar system, taking into account modern scientific views [6, 7, 8, 18,
21].

Patients with ankylosis, post-traumatic condylar destruction, advanced degenerative disease and multi-operated patients are those who require joint replacement. Rise in development of biocompatible materials, improved designs for patient-fitted prostheses have been made with emerging advances. These devices can be used to treat internal derangement cases with history of multiple surgical and nonsurgical treatment failures, as well as restoring form and function following the removal of failed Vitek Proplast-Teflon containing temporomandibular joint implants. This paper will provide an insight related to treatment modality in debilitated, functionless temporomandibular joint (TMJ) patients.
