Primary Hydatid Cyst of Breast: A Case Report

Hydatid cyst is a disease which is caused by the larval form of Echinococcus granulosus and seen endemically among sheep-raising communities. The larvas penetrate the bowel mucosa and arrive in the liver via the portal vein. Most common sites of cyst formation are liver and lung and 10% in other organs. Hydatid cyst of the breast accounts for 0.27% of all cases only. We report a case of 35 year old female presented to the surgical OPD with complaint of lump in left breast since 5 years, there was no other associated complaint. Fine needle aspiration cytology revealed features of hydatid cyst, the lump was subsequently excised under local anaesthesia.

Correlation between histopathological results and BIRADS classification in breast masses

Background: Breast cancer incidence in India is increasing and has now become the most common cancer among women. Preoperative pathology diagnosis and mammography (using breast imaging reporting and data system (BI-RADS) scoring system) constitute an essential part of the workup of breast lesions. The present study was aimed to compare the diagnostic accuracy    of BI-RADS score with histopathological finding in diagnosis of benign and malignant lesions of breast.
Methods: This is a cross-sectional study with study duration of 1.5 year (January 2021- June 2022) The present study was conducted on 100 randomly selected newly diagnosed cases of breast  masses.
Results: Considering histopathological examination as gold standard, the sensitivity and specificity of BI-RADS score is 93.9% and 82.8% respectively. The positive predictive value, negative predictive value and diagnostic accuracy of BI-RADS score is 91.04% and 87.8 % respectively.
Conclusions: The inference derived from the present study is that BIRADS is a very useful tool specially owing to the fact that it is noninvasive, which leads to lesser trauma and faster report however the tissue diagnosis using H&E and other stains remains the gold standard and should always be restored to before undertaking surgery.

Diagnostic Accuracy of Ultrasound Guided Core Needle Biopsy in Various Breast Lesions

Background: Breast cancer is the most common female cancer worldwide representing nearly a quarter (25%) of all cancers. Diagnosis of breast cancer is carried out by three medical members, the surgeon, who covers breast examination, the radiologist for mammography and ultrasound, and the pathologist for fine-needle aspiration cytology (FNAC) and core needle biopsy (CNB). Aim of present study is to assess the accuracy of ultrasound guided core needle biopsy in various breast lesions
Material & Methods: A hospital based prospective study done on 100 cases of various breast lesions reporting to the Surgery Department, J.L.N. Medical College and Associated Group of Hospitals, Ajmer, Rajasthanwithin study duration and eligible as per inclusion criteria were included in the study from January 2020 to December 2021. Then an ultrasound guided biopsy specimen is obtained by means of four successive insertions with different angulations of the needle into the core of the lesion. The quantity and quality of the material obtained is judged after immediate immersion of the specimen in fixative, and then specimen is sent to histopathology department. And the core needle biopsy report is compared with the excision biopsy report of the breast lump.
Results: Our results showed that the mean age was 38.79 ± 13.98 yrs (age range 18-75 yrs). On USG guided core needle biopsy and excision biopsy, 53% were found benign tumor, and 47% were carcinoma. Sensitivity is 100% and specificity is 100%. There was no false positive and false negative case present in this study.
Conclusion: We concluded that Ultrasound guided Core needle biopsy can be reliably used for preoperative diagnosis of breast lumps as the first diagnostic step with high sensitivity, specificity, and accuracy for both malignant and benign lesions. However, benign lesions, diagnosed this way, must be followed by frequent examinations.

Clinical profile of patients with benign breast disease

The nomenclature of benign breast disease is very confusing.This is because over the last
century a variety of clinicians and pathologist have chosen to describe a mixture of
physiological changes and disease processes according to a variety clinical, pathological and
aetiologicalterminology. The study includes patient attending surgical out patient department
and admitted to surgical wards with breast lump during the study period. Most of patients
presented in OPD with complaints of breast lump 66 (60%) followed by patients presenting
with lump and pain of 33(30%). Some patient show engorgement of breast 8 (7.2%).Few
Patients also presented with lump, pain and discharge 3 (2.7%).
